Physical Therapist Location: Elizabeth, WV Agency: Windsor Healthcare Recruitment Group, Inc. Pay: Competitive weekly pay (inquire for details) Shift Information: Days - 5 days Start Date: ASAP About the Position Notes updated 8/25/2025 - MS Home Health Gassaway - 100 Hoylman Drive Gassaway, WV Department: PT of patients treated per day: 5-6 units (SOC=2, Discharge=1, Recert=1.5, Regular visit=1) General Certification Requirements BLS Other Certs Required for all PT: Any PT requirements for practicing in the state of WV Common Dx/Types of patients: General ortho (hips, knees), generalized weakness, strengthening, cardiac rehab Acute Inpatient, Acute Rehab, Skilled, Home Health, Outpatient? All our patients are seen in the home health setting What areas are they required to cover: We currently have 8 counties (Braxton, Gilmer, Calhoun, Nicholas, Webster, Clay, Randolph, Pocahontas) we would work with the therapist depending on where they are living to do the best we could for mileage/productivity. We currently have a therapist that lives in Randolph and Pocahontas, so those areas are covered. Ideal would be someone helping to cover Webster/Nicholas Counties, but we are willing to work with whatever we have available Experience or Must Have skills & common job duties: General duties listed under PT job description; open, positive attitude about learning Epic system and doing their best to provide excellent care to our patients PT Manager/Supervisor: Kelli Gregory and Lacy Hamrick Rehab aides: We currently have 4 PTAs Shifts & Scheduling: (Do you allow self-scheduling? Block scheduling?) We do allow self-scheduling. PTs are free to move patients around on their schedule according to area if it meets the initial and reassessment guidelines Weekend requirements: No weekend requirements Holiday requirements: No holiday requirements Scrub Color/Dress Code Navy Scrubs or Khaki bottoms with navy polo Department specific Orientation Orientation would take place at the Gassaway office with learning Epic documentation; each orientation is tailored to clinician need obviously with a travel contract we would want that to happen as soon as possible.
