provides medical, behavioral, and therapeutic supports for
individuals with intellectual and developmental disabilities in
Pike and Scioto counties. Our mission is to enhance quality of life

Communication, and Passion. Accountability Objectives: The Case
Manager is responsible for training and supervising Team Leads and
Direct Support Professionals to ensure effective implementation of
services and programs for individuals on their caseload. This
includes assessing, developing, implementing, and reviewing plans
to ensure all services are delivered according to each person's
Individual Service Plan (ISP). The Case Manager provides
comprehensive oversight and care coordination across all areas of
each individual's life, including staffing, medical needs, service
delivery, and overall quality of care. They ensure programming
compliance and proper supervision in accordance with Ohio
Department of Developmental Disabilities regulations, the County
Board of DD, and all applicable federal requirements. Additionally,
the Case Manager ensures adherence to Scioto Trails policies and
procedures, maintains ongoing regulatory compliance, and ensures
that all resident needs are consistently met. They are responsible
for the effective operation and management of their assigned
caseload within the Waiver Department. Minimum Requirements:
Associate's or Bachelor's degree in Human Services preferred. At
least 1 year of experience working with individuals with
developmental disabilities (2 years preferred). Ability to use
discretion and independent judgment in oversight and service
delivery. Valid Ohio Driver's License and insurable to drive
company vehicles. Loss of license/insurability may affect
employment. Ability to provide therapeutic interventions during
crisis situations and communicate effectively with verbal and
non?verbal individuals. Willingness to work collaboratively with
families, guardians, community partners, and service providers.
Must pass required criminal background check, drug test, and
reference checks per state and federal Waiver regulations. Must
complete and maintain Medication Certification and any
task?specific delegations as required. Scioto Trails/Advancing
